If you have forgotten your password, you must contact the Online Services Helpdesk (OLS-HD) to be issued a temporary password.
Select Help from the menu at the top of the screen to find the contact information for the OLS-HD.
Telephone the OLS-HD, or complete the Support request form to request a temporary password.
When you have received your temporary password by email, use it to login to the FDSR system.
You will be prompted to change your password before continuing in the system.
